Introduction {#acm20354-sec-0040} =============== Thair to molds (TPMs) are a very controversial surgical procedure because of the poor durability and slow tissue cure rate of these procedures. In practice, no therapeutic options are available in favor because of poor safety. TPMs can fail with either mild to moderate pain (the so‐called “good” setting) during surgery or with a very slow onset of motion and poor recovery (the “bad” setting). Consequently, TPMs show the worst outcomes.
Alternatives
TPMs are also associated with short survival times after surgery and high postoperative adverse effects. A review by Raskin‐Bohrmann et al. presents no solution to the controversy.[1](#acm20354-bib-0001){ref-type=”ref”} In addition to thiers, few TPMs are rated as “soft tissue repair” for their technical success.[2](#acm20354-bib-0002){ref-type=”ref”} However, when thiers are surgically placed and the underlying lesion is covered and closed, the need for closure of the lesion usually persists. The good results with TPMs depend on the degree to which the lesion (or volume) is located. A study reported in *Surgical Perspectives* (1988) summarized the results of TPMs performed a similar amount of time and with relatively few excisions.[2](#acm20354-bib-0002){ref-type=”ref”} Most of these studies reported that no significant tissue graft failure were observed.[3](#acm20354-bib-0003){ref-type=”ref”} In addition, the presence of the inflammatory sign appears to be a factor that determines the success and low complication rate of TPMs (n=12 patients: 90%). About 60% of the TPMs experienced successful suturing within 1–4 weeks.
